日期: 10-4-2008致：2008年1月份/4月份注册新生/4月份升班生 健康检查 尚未进行健康检查的2008年1月份/4月份注册新生/4月份升班生(已缴付RM35 健康检查费者)可在4月30日前自行安排时间，前往学院指定的诊所Polyclinic Sentosa ,完成健康检查。 诊所营业时间： 周一至五：8:30 am 至9.00pm 周六：8:30 am 至4.00pm 周日9:00 am 至1.00pm 诊所地点：与东升茶餐室同一排，最尾端。 请携带学生证、已填妥的健康检查表格同行，如未取得表格，可直接到诊所填写。 逾期者未完成健康检查者，学院保留以2008年4月份学期学期末扣考为处分的权利。 注册处 谨启 To: 2008 JAN Intake/April Intake/Raising Students Medical Check Up for late comers For student who has registered but yet to complete the medical check up process during the registration day kindly proceed to Polyclinic Sentosa latest by 30th APRIL 2008 to carry out the medical check up. Clinic Business hour: Mon- Fri: 8.30am to 9.00pm Sat: 8.30am to 4.00pm Sun: 9.00am to 1.00pm Venue: Polyclinic Sentosa: Opposite of theChurch, same row as Kedai Makanan Dong Sheng. Kindly bring along the your student ID, MedicalReport Form during the check up, you can collect the said form from the appointedclinic if you do not have one. For those who failed to complete the medicalcheck up on the due date, the college reversed rights to bar the student fromsitting for the April semester finalexam. Thank you.
